1) By submitting this form you agree and authorise JC Injectables to charge your credit card the nominated weekly or monthly ammount on eather a weekly basis for 52 weeks, or on a montly basis for 12 months on the nominated weekday, commencing no later than 7 calendar days after submitting this form.

2) In the event you would like to cancel your Membership Plan, you agree to provide JC Injectables with 2 weeks written notice of such request via email on contact@jcinjectables.com. The cancelation date will be 14 calendar days after notice is received.

3) Upon cancelling your Membership Plan, you agree and authorise JC Injectables to charge any outstanding amount for treatments received in advance, so that, your account balance is zero on the nominated cancelation date.

4) In the event your account is in credit following the cancelation of your Membership Plan, JC Injectables will refund the amount to the original form of payment within 7 calendar days after the cancelation date.

5) In the event you miss a payment, you agree to pay any outstanding contributions prior to receiving your next scheduled treatment otherwise, JC Injectables may decline to offer treatment services to guests.
